Always slide your FREEZE SLEEVE on with the wide end at the top. The black threading should always be at the bottom on both your knee or elbow.
You want the sleeve to be tight and have compression, however if it is too tight (i.e. too small) the sleeve will overstretch. When the sleeve is too small it will loose it's cold therapy too fast and not provide optimal results.
Likewise, if the sleeve is too loose (i.e. too large) you will not receive the compression therapy along with the cold therapy. Also, when you get up and move around your sleeve should not slip off.
